Means of Referral

We are no longer accepting referrals through Ocean. Please refer via fax to (519) 741-0286.

Required in every referral:

  • Preferred service or provider (pediatrics, respirology, or lactation)

  • Complete demographics/CPP, ideally including e-mail address

  • Special considerations (need for translation, unsafe persons, pronouns)

  • Goal of referral (advice, co-management, 2nd opinion, diagnostic clarification)

  • Reason for referral

  • Brief summary of history/work up/investigations

  • Current medications

  • *Growth charts if applicable (e.g. growth, development)

*referral will be rejected if missing required information

Important Notes

Urgent Referrals: Kidwell Pediatrics is NOT able to accept urgent referrals at this time. Please call the pediatrician on call at Waterloo Regional Health Network (WRHN) for urgent matters for patients in KW, or your local hospital-based pediatric service if the patient is not in KW.

Shared Care Model: The pediatricians at Kidwell cannot generally offer urgent appointments due to competing responsibilities at the hospital. They are usually in clinic ~1-2 days per week. We rely on our excellent primary care colleagues to offer sick visits and assessment to our shared patients. We appreciate PCPs who copy us on updates when seeing our shared patients for these reasons.

Primary Care: The pediatricians at Kidwell DO NOT provide primary care, or vaccines. We offer consultation for specific medical, psychological, behavioral, or developmental problems. Referrals for primary care will thus be rejected. Once specific problems are resolved, patients are discharged from care.

Pediatric Respirologist

Accepting referrals for pediatric respirology (e.g. asthma, chronic cough, shortness of breath, wheezing, recurrent chest infection, recurrent croup, home oxygen requirements, etc.) and pediatric sleep medicine (e.g. sleep apnea, insomnia, adolescent sleep problems, behavioral sleep associations). Referrals for spirometry alone will be rejected.

Cara MacRae, MD, FRCPC

Pediatric Consultant

Accepting referrals for pediatric medicine (e.g. failure to thrive, neonatal problems, constipation, skin problems, etc.) and developmental pediatrics/school problems (e.g. ADHD, learning difficulties, behaviors).

Rebekah Baumann, MD, FRCPC

Pediatric Consultant

Not currently accepting referrals.

Faith Koudijs, RN, ICBLC

Lactation Consultant

Accepting referrals for breastfeeding problems, counselling, and support. This is OHIP-covered through a referral to Dr. MacRae.
